These days we are so busy that we rarely get a chance to visit the doctor. When we finally schedule an appointment, we expect the doctor to immediately recognize all of our problems and fix them. Unfortunately, medicine doesn't work that way. But, this certainly doesn't mean that your problems can't be solved. They can and will be addressed in a thorough and efficient manner if you come prepared.
To prepare for your appointment, we suggest that you review our Appointment Prep Guide and jot down any information that you would like the doctor to know. If you are a new patient, we also recommend that you fill out the Patient Information, Medical History, and Medications List forms before arriving at the office. This will speed up the mandatory processing of your file and you will get to see the doctor much more quickly. If you are a returning patient with hypertension or diabetes, you may find the Blood Pressure and Blood Sugar charts useful for monitoring your condition.
